Welcome to our New Patient Registration Process

You now have the opportunity to fill out all of your paperwork online.

 

 

Here's How it Works:

1. When you click the link at the bottom of the page you will be directed to our privacy policy.
2. You should then read the policy, and check the box if you agree to the terms.
3. It then takes you to our financial policy, you must also agree to this policy to continue.
4. The next page is the actual registration page.  Here you need to complete all of the fields that are applicable.
5. BEFORE you click submit read over all of the information you have entered.
6. The last and final page will display the information in a printable form.  You must PRINT this form
7. If you don't have a printer click FILE, SAVE AS, and save it to Some type of removable media
   (flash drive, compact disc, etc. and bring it with you to your first appointment)
